Occlumency is really badly described. I mean, the 'clear your mind' stuff reminds me of meditation, but...that's not the most practical thing to do when someone's aiming spells at your head ala Snape (he could just be a really bad teacher, though). Doing it before bedtime makes sense, though, so...yeah. Who knows? I do subscribe to the theory that more people than just Snape and Dumbledore know it, and that Dumbledore chose Snape purely because he had a reason to be in the school.
I thought it was every time you killed, but - if you didn't use the death to make a Horcrux - the soul eventually healed itself over again, while if you did make one then the split was permanent. Of course, that could just be me mistaking headcanon for canon again. It happens sometimes...
